Community Support Services (CSS)

Community Support Services (CSS) are provided to adults with a mental health
diagnosis and related functional impairments. Community support services is a
Medicaid program designed to increase integration and stability in the community. Our
CSS team works with individuals to provide the following support:

  •  Monitoring of mental health symptoms
  • Communication with other providers
  • Appointment attendance
  • Medication management
  • Crisis intervention
  • Development of natural support
